Diabetes Self-Management Program
Joann Webster, RN, Certified Diabetes Educator for Multicultural Home Care created a Diabetic Program for patients living with Diabetes that consist of self-management skills focusing on the AADE7 self-care behaviors, as well as diabetes case management. We are dedicated to helping people manage their diabetes. Joann currently works as a visiting nurse and staff educator with Multicultural Home Care specializing in diabetes care and chronic care management.
Multicultural Home Care encourages people living with Diabetes to "Test! Don't Guess" when it comes to managing their blood sugar.
MCHC recognizes that Diabetes is an Epidemic happening across all cultures. Our Certified Diabetic Educator & Chronic Care Team focuses on diabetes education for the patient and family members in the comfort of their home.
Focusing on:
- Educating patients on techniques for risk reduction of acute and chronic complications.
- Training patients and caregivers to use diabetes devices for better self-management.
- Motivating patients to set and achieve smart behavior-change goals.
- Providing nutritional education including diabetes meal planning.
- Nurses who speak English, Spanish, Cambodian, Chinese, Vietnamese, Russian, Haitian-Creole and Portuguese are able to provide culturally sensitive diabetes education.
- All diabetic patients are given a valuable MCHC diabetic handbook available in English, Spanish and Chinese with a 5th grade literacy.

MCHC main office is bathed in blue light for World Diabetes Awareness Day November 14th.
National Certification Board Diabetes Educators was established in 1986 to develop and administer a certification program for health care professionals who teach individuals with diabetes how to manage their disease.
Through the development, maintenance, and protection of the certification process and the CDE credential, NCBDE recognizes and advances the specialty practice of diabetes education.
NCBDE supports the concept of voluntary, periodic certification for all diabetes educators who meet credential and experience eligibility requirements.
